By booking, you agree to receive care by telehealth and you confirm you are located in Florida, Washington, New York, or Arizona at the time of your visit.
Cash-pay / no insurance
Primary Pathway Health is a cash-pay practice. We do not bill insurance. Payment is due at the time of booking. You will receive a receipt you may submit to your insurer if you have out-of-network benefits.
