Why this policy exists
Washington's My Health My Data Act gives Washington residents extra protections over their consumer health data. This page explains what we collect, why, and your rights. It supplements — it does not replace — our Privacy Policy and our HIPAA Notice of Privacy Practices.
What we collect
When you use our callback form, we collect your name, phone number, email address, preferred contact method, and preferred callback time — so we can reach you about scheduling. We do not collect health details on this website, and we ask you not to include them.
Your consent
We collect the above only with your opt-in consent, given by checking the consent box on our callback form. The box is unchecked by default — nothing is collected until you check it.
How we use it
Only to contact you about scheduling, as you requested. We do not sell consumer health data. We do not share it with third-party advertisers or data brokers.
Withdrawing consent
You can withdraw your consent at any time by emailing bertine@primarypathwayhealth.com or calling/texting (321) 399-2486. Tell us you withdraw your health-data consent and we will stop using your information for outreach and delete it from our callback list.
Your other rights
Washington residents may also request to know what consumer health data we hold about them, and to correct or delete it. Contact us at the email or number above to exercise these rights.
No geofencing
We do not use geofencing around health care facilities.
